Adult rehabilitation
Rehabilitation is a goal-oriented dynamic process, which enables individuals with impairments, activity limitations, and/or participation restrictions to identify and reach the optimal physical, mental and/or social functional level. This is achieved through client-focused partnerships with families, providers and the community.
Rehabilitation focuses on abilities and aims to facilitate independence and social integration
The adult Rehab service aim to help people return to independent living. The service enables people with severe and complex health problems to gain or regain the confidence and skills in everyday activities, which will enable them to fulfill their potential for recovery and independence.
Adult Rehabilitation beneficiaries:
v Neurological condition - e.g. stroke
v Musculo-skeletal trauma - e.g. amputations and Motor Vehicle Accidents
v Neuro-muscular - e.g. Multiple Sclerosis
v Nervous system - e.g. Guillain-Barre Syndrome
v Other medical conditions warranting rehabilitation
Adult Rehabilitation Team
v Doctor, for consultation & examination.
v Nurses, will help you with your nursing and medical care as needed.
v Psychosocial, will provides support the beneficiaries and family.
v Physiotherapist will teach you exercises to help you get around and increase your strength.
v Occupational Therapist will help you with exercises to make you stronger. They will teach you new ways to do things like washing, dressing and cooking for yourself. They will visit your home, if needed, to get it ready for you when you leave the program.
v Dietitian will look at your nutrition status and educate as required.
v Speech Language Pathologist will help you with your speech if you need it.
*Our multidisciplinary teams support service users and their relatives to identify and work towards their goals for recovery
